Respect Aged Care is seeking a part time compassionate and dedicated Social Care Coordinator to join our team in our Cohuna facility in VIC. This is a unique opportunity to contribute meaningfully to the lives of our residents by coordinating and delivering social, emotional, and recreational support. The ideal candidate will have a person-centred approach, be a strong communicator, and have a passion for enhancing the quality of life for older people.

This role is a minimum of 28 hours per week; days can be negotiated if required.

Key Responsibilities:

• Plan, coordinate, and implement lifestyle and social care programs tailored to individual resident needs

• Foster a supportive and engaging environment through meaningful activities and therapeutic services

• Build positive relationships with residents, families, and the wider community

• Maintain accurate documentation and contribute to care planning

• Work collaboratively with the broader care team to promote holistic wellbeing

Essential Requirements:

• Certificate III in Leisure and Lifestyle
• First Aid Certificate.
• Proficient computer systems and information management skills.
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a multidisciplinary team
• Understanding of the aged care quality standards and person-centred care principles
• National Police Check and relevant working rights in Australia

Desirable Requirements:

• Experience in the provision of complementary therapies and recreational services

• Demonstrated experience in dementia care

• Experience in residential aged care settings

• Knowledge of and connections with local community groups and support agencies
