About the role
As the Social Care Coordinator, you will play a vital role in supporting the emotional, social, and psychological wellbeing of our residents. You will design and deliver engaging programs and activities that foster connection, independence, and quality of life. Your compassionate approach and ability to form genuine relationships will help residents feel valued, included, and at home.
Key responsibilities include:
Planning, coordinating, and facilitating individual and group lifestyle programs
Building meaningful relationships with residents to understand their interests, histories, and goals
Encouraging social engagement and participation in daily activities
Supporting residents transitioning into the home to feel welcome and connected
Maintaining accurate documentation, lifestyle assessments, and care plans
Collaborating with the care team, volunteers, families, and community groups
Ensuring all programs align with the Aged Care Quality Standards and resident-centred care principles
About you
You are compassionate, energetic, and committed to enhancing the wellbeing of older people. You enjoy bringing creativity and positivity into your work and thrive in a role where every day is different.
You will bring:
Experience in a lifestyle, activities, leisure and health, social care, or related role
Certificate IV in Leisure and Health or willingness to work towards the qualification
Excellent communication, relationship-building, and organisational skills
A warm, engaging approach with the ability to motivate and connect with residents
Strong teamwork skills and the ability to collaborate across departments
Current National Police Check or willingness to obtain one
Full COVID 19 vaccination in line with industry requirements
