We are currently seeking a Care Manager to lead and support the clinical care team at our St John’s Aged Care Facility in Wangaratta, Victoria. As the Care Manager, you will play a pivotal leadership role in overseeing the day-to-day clinical operations of the facility. Working closely with the Facility Manager and multidisciplinary team, you will ensure the delivery of safe, high-quality care and services aligned with our organisational values and aged care standards.
Lead and manage clinical care staff to deliver exceptional resident outcomes
Monitor and maintain compliance with all clinical governance and regulatory requirements
Foster a positive team culture that supports staff development and resident wellbeing
Liaise with residents, families, and health professionals to ensure individualised care plans are developed and implemented
Drive continuous improvement in care practices and service delivery
Support staff education and training to ensure clinical competencies are maintained across the team
Conduct regular audits and assessments to monitor resident outcomes and drive continuous quality improvement
Collaborate with the wider Respect network to share best practices and contribute to organisational initiatives
Current registration with AHPRA as a Registered Nurse
Previous experience in a similar leadership or management role within aged care
Strong understanding of Aged Care Quality Standards and accreditation processes
Excellent communication, leadership, and organisational skills
A passion for aged care and commitment to person-centred care
Proficient in clinical documentation, care planning, and the use of aged care software systems (e.g. AutumnCare, LeeCare, or similar)
Demonstrated ability to manage competing priorities and adapt to a dynamic aged care environment
