About the Role
The Care Manager is responsible for coordinating and delivering high-quality clinical care in line with contemporary practice, ensuring care is planned, allocated, and delegated in accordance with documented policies and procedures. This role contributes to the effective management of human, material, and financial resources while supporting safe, efficient, and person-centred care delivery.
Originally opened in 1995, Yallambee Lodge offers 40 private rooms, each with its own ensuite. The home provides a comprehensive range of care services, including specialised palliative care, to support residents with complex personal and medical needs. Yallambee Lodge is also well equipped to support residents requiring all levels of care.
No two days are the same! As a Care Manager, you will:
- Work collaboratively within a team to deliver high-quality care, while supporting, mentoring, and sharing knowledge.
- Uphold duty of care by complying with policies, professional and legislative standards, supporting resident and staff rights, managing risk, and maintaining professional practice through ongoing development.
- Support effective administration through policy input, compliance monitoring, and active participation in audits, reviews, and quality improvement activities.
- Foster a positive, high-performing work environment by leading and supporting teams, driving best-practice care and overseeing care planning.
- Support staff development by participating in performance reviews, mentoring and upskilling team members, and collaborating with leadership to develop, deliver, and review education programs.
- Build and maintain strong relationships with external providers and community services, while staying informed and responsive to developments in aged care, nursing, and community health.
About You
You’ll thrive with us if you have:
- Current AHPRA registration as a Registered Nurse
- Strong organisational skills and attention to detail
- Strong leadership and communication skills
- Age care management experience
- Sound clinical knowledge and skills in accordance with contemporary practice.
- Ability to build positive relationships with residents and staff
- Well-developed initiative and a positive, constructive attitude
- Current National Police Certificate (or ability to acquire).
