Care Partner – Home Care

Based from our Devonport Office | Supporting the local community

Join a purpose-driven organisation delivering high-quality home care and community services across Northern Tasmania. We’re seeking a compassionate Care Partner, Home Care to support older people living in the local community, based from our Devonport office.

In this role, you’ll provide person-centred, relationship-based case management, supporting clients with moderate to high needs to live safely, confidently, and independently at home. You’ll work closely with clients, families, and local services, coordinating supports, monitoring wellbeing, and responding to changing needs through regular face-to-face and phone contact.

This is a hands-on, community-connected role for someone who values building trust, understands the local area, and wants to make a meaningful difference close to home.


Key Responsibilities

  • Deliver safe, person-centred, enablement-focused case management to clients in the local community.

  • Conduct comprehensive assessments of clients’ functional, social, environmental, and wellbeing needs.

  • Develop individualised care plans and goals that promote independence, choice, and quality of life.

  • Maintain regular contact with clients through a combination of home visits, phone, and virtual check-ins.

  • Proactively monitor wellbeing and identify early changes in client condition or circumstances.

  • Escalate risks, incidents, or emerging needs to Clinical Care Partners or Managers as required.

  • Coordinate referrals to clinical staff, allied health professionals, community supports, and external providers.

  • Support smooth transitions into and out of Restorative Care Pathways, including handovers and follow-up.

  • Work collaboratively with scheduling and workforce teams to ensure reliable, consistent service delivery.

  • Maintain accurate, timely, and compliant documentation, care plans, and case notes.

  • Participate in multidisciplinary discussions, case conferences, and high-risk reviews.

  • Provide culturally safe, inclusive, and respectful support to CALD, ATSI, LGBTQIA+, and diverse communities.


Requirements

Essential

  • Minimum 3 years’ experience in case management, aged care, home care, or a related discipline.

  • Sound knowledge of the home care sector, including Aged Care Quality Standards and the Aged Care Quality Code of Conduct.

  • Strong communication skills with the ability to build trusting relationships with clients and families.

  • Participant-centred mindset with strong advocacy skills.

  • Proven ability to manage a varied caseload, prioritise effectively, and work autonomously.

  • Experience with budgeting and home care funds management.

  • Understanding of hazard identification, incident reporting, and risk management processes.

  • Competency using client management systems and confidence with technology.

  • Well-developed problem-solving, decision-making, and conflict resolution skills.

  • Current National Police Certificate (or ability to obtain).

  • Valid Driver Licence and willingness to travel locally.

  • Based in Tasmania, with strong knowledge of the local community.

Desirable

  • Certificate IV in Home & Community Care, Aged Care, or a related qualification.

  • Bachelor’s Degree (Registered Nurse desirable but not required).

  • Exposure to workforce planning or scheduling systems.
