We are seeking a dedicated and experienced Clinical Care Coordinator to join our team at our Toogoolawah facility on a part-time basis. Nestled in the picturesque Brisbane Valley, Toogoolawah offers a relaxed country lifestyle, welcoming community spirit, and easy access to both Brisbane and the Sunshine Coast. About the Role

As Clinical Care Coordinator, you will play a vital role in ensuring our residents receive the highest standard of person-centred clinical care. You will:

  • Assess resident needs and develop tailored care plans.

  • Lead and support a dedicated care team to deliver safe, high-quality care.

  • Maintain accurate clinical documentation and oversee compliance requirements.

  • Provide hands-on nursing care, including wound management, medication administration, and monitoring.

  • Communicate effectively with residents, families, and health professionals.

  • Support infection control measures, clinical audits, and quality improvement initiatives.

  • Act as a role model and mentor for nursing and care staff.

About You

We are looking for someone who brings:

  • Bachelor of Nursing and current AHPRA registration.

  • Experience in residential aged care nursing.

  • Strong clinical assessment, documentation, and leadership skills.

  • Excellent communication and interpersonal skills.

  • Computer literacy, including the use of Microsoft Office and clinical software.
