About the Role

Wattleglen is a new and contemporary residential aged care home where exceptional care is delivered within a peaceful and welcoming community environment. As a Clinical Care Coordinator, you will play a pivotal leadership role in ensuring the delivery of high-quality, person-centred care within our aged care home. Working full time, you will oversee clinical governance, coordinate care planning, and support a multidisciplinary team to meet residents’ complex clinical needs. You will provide clinical guidance and mentorship to nursing staff, ensure compliance with aged care standards and legislation, and work closely with management, residents, and families to promote safe, effective, and compassionate care outcomes.

No two days are the same! As Clinical Care Coordinator you will:

  • Coordinate the admission of new residents, completing comprehensive assessments and developing person-centred care plans in collaboration with healthcare professionals.
  • Lead, mentor, and support the care team to deliver safe, high-quality clinical care and positive resident outcomes.
  • Provide hands-on nursing care, including medication administration, wound management, and ongoing clinical monitoring.
  • Maintain accurate clinical documentation, oversee infection control practices, and ensure compliance with regulatory standards and audits.
  • Communicate effectively with residents, families, and external healthcare providers while driving quality improvement initiatives.

About You

You’ll thrive with us if you have:

  • Bachelor of Nursing with current AHPRA registration.
  • Minimum 2 years’ nursing experience in residential aged care, with experience in a similar clinical care role highly regarded.
  • Demonstrated ability to work collaboratively within multidisciplinary care teams.
  • Strong clinical assessment, documentation, and evidence-based practice skills.
  • Excellent communication and interpersonal skills to engage effectively with residents, families, and colleagues.
  • Proficient computer and information management skills, including Microsoft applications and clinical software systems.
  • Sound understanding of the aged care accreditation process and ability to meet Aged Care Worker Screening legislative requirements.
