As a Registered Nurse – Home Care, you will provide high-quality, person-centred clinical care to participants across Hobart and surrounding areas. This role offers a high level of autonomy, diverse day-to-day experiences, and the chance to develop strong, meaningful relationships with participants and their support networks.

This is a part-time position requiring availability for three days per week, from 8:30 am to 4:30 pm, offering a consistent schedule that supports work–life balance.

We are seeking a nurse who is committed to delivering holistic, culturally responsive care and upholding safe, evidence-based practice, within a supportive team environment and a strong clinical governance framework.

No two days are the same! As Registered Nurse – Home Care you will:

  • Conduct comprehensive clinical assessments and develop, implement, and review individualised care plans in collaboration with participants and the multidisciplinary team.
  • Deliver a range of complex nursing care in the home, including wound care, medication management, chronic disease management, and end-of-life support.
  • Monitor participant health, respond to changes, and escalate clinical concerns in line with governance frameworks.
  • Collaborate with Care Partners, allied health, GPs, and external providers to ensure coordinated, person-centred care.
  • Provide education and support to participants, families, and support workers, promoting safety, independence, and wellbeing.
  • Maintain accurate clinical documentation and ensure compliance with professional standards, legislation, and quality and safety requirements.

About You

You’ll thrive with us if you have:

Essential:

  • Current registration as a Registered Nurse with AHPRA with no restrictions.
  • Minimum two years post-registration clinical experience, ideally in aged care, community nursing, or home care.
  • Competence in clinical assessment, care planning, and delivery of complex nursing care in community settings.
  • Knowledge of the Aged Care Quality Standards, relevant legislation, clinical governance, risk management, and incident reporting.
  • Current driver’s licence, First Aid/CPR certification, and ability to travel to participants’ homes.
  • Strong communication skills and ability to maintain accurate, compliant clinical documentation.

Desirable:

  • Experience with home care frameworks (Home Care Packages, CHSP, Support at Home) and supporting participants with complex clinical needs, including dementia.
  • Familiarity with electronic clinical systems and participation in quality improvement or clinical governance activities.
  • Postgraduate or specialist training in aged care, wound care, chronic disease management, or palliative care.
