About the Role
Lead a Team That Makes a Real Difference!
As our Team Leader – Care Management Services, you'll lead a dedicated team of Care Partners delivering person-centred support through the Support at Home program. This is an opportunity to shape the experience of older Australians, ensuring they receive the right services at the right time to live independently, safely and well.
In this hands-on leadership role, you'll coach, mentor and empower your team to deliver exceptional outcomes for participants. You'll provide day-to-day guidance, oversee service quality and caseload management, and help drive a culture focused on wellness, reablement and continuous improvement. Working closely with Clinical Services, Workforce Planning & Scheduling and other key stakeholders, you'll play a pivotal role in ensuring services are coordinated, compliant and responsive to participant needs.
Based in Launceston, Devonport or Hobart, this is a full-time Monday to Friday position offering the opportunity to combine operational leadership with meaningful impact.
We're looking for an experienced people leader with a background in aged care, home care, community services or a related human services environment. You'll bring a proven ability to lead and develop teams, build strong relationships and foster a collaborative, solutions-focused approach. Your understanding of the Support at Home program, person-centred care, wellness and reablement principles, and the Aged Care Quality Standards will enable you to confidently guide your team through a period of significant sector change while delivering outstanding participant outcomes.
