Team Leader- Case Management Services

We are seeking an experienced and passionate Team Leader to join our dynamic team. In this leadership role, you will coordinate and oversee the Home Care Package Program, ensuring the delivery of high-quality, client-centered care. You will work collaboratively with the Case Management team and other key stakeholders to ensure that services are provided effectively, efficiently, and in alignment with the client’s goals.

Key Responsibilities:

  • Lead and coordinate the Home Care Package Program, working in conjunction with senior management to ensure effective delivery of services.
  • Lead the case management model of care, monitoring client outcomes and service effectiveness.
  • Monitor package expenditure to ensure services are provided within budget and Home Care Package guidelines
  • Contribute to marketing efforts to drive growth in Home Care Packages.
  • Identify and implement service improvements to enhance care quality and competitiveness.
  • Build and maintain relationships with key stakeholders, including government agencies, healthcare providers, and community organisations
  • Ensure compliance with data collection requirements and funding targets.
  • Actively manage compliments, complaints, and feedback, striving for continuous improvement

Requirements:

  • Minimum of 3 years’ experience in aged care, home care, or a related discipline.
  • Sound knowledge of the Home Care and/or Aged Care industry, including the Aged Care Quality Code of Conduct, Aged Care Standards, and accreditation processes.
  • Strong communication skills with the ability to build rapport with internal and external stakeholders
  • Experience in budgeting and fund management related to Home Care Packages
  • Willingness to travel both intra and interstate (roughly pnce per Month)
  • Availability to participate in after-hours on-call duties
  • Based in Tasmania (Launceston Preferred) or NSW

The successful candidate will receive an annual salary in the range of $95,000 - $103,000 exclusive of super.
