About the Role

We are seeking an experienced Virtual Registered Nurse to provide clinical oversight and support across our Home Care programs. This full-time role can be based in either our Launceston or Hobart office.

Working collaboratively with Care Partners, Scheduling teams, and operational staff, you will conduct clinical assessments and reviews, support participant follow-up, monitor documentation, contribute to incident management, and drive quality improvement initiatives. Through proactive virtual engagement, you will help identify risks, coordinate timely interventions, and support the delivery of safe, person-centred care.

No two days are the same! As Virtual Registered Nurse – Home Care you will:

  • Conduct virtual clinical assessments, reviews, and participant follow-up to support safe, person-centred care.
  • Complete post-hospital discharge reviews and support effective care transitions.
  • Identify clinical risks, changing participant needs, and coordinate appropriate interventions and escalations.
  • Provide clinical guidance and support to Care Partners and operational teams within scope of practice.
  • Maintain accurate clinical documentation and monitor records to ensure compliance with clinical and organisational standards.
  • Support incident management, investigations, audits, and quality improvement initiatives.
  • Collaborate with multidisciplinary teams, health professionals, participants, and families to ensure continuity of care.
  • Contribute to clinical governance, regulatory compliance, and the delivery of best-practice aged care services.

About You

You’ll thrive with us if you have:

Essential:

  • Current registration as a Registered Nurse with AHPRA, with experience in aged care, community care, home care, disability, or a related healthcare setting.
  • Demonstrated experience conducting clinical assessments, participant reviews, care planning, intake, onboarding, and ongoing service delivery.
  • Strong clinical knowledge with the ability to identify, assess, and respond to participant risks, concerns, incidents, safeguarding matters, SIRS, and escalation requirements.
  • Sound understanding of person-centred practice, wellness and reablement principles, contemporary care management models, and risk management frameworks.
  • Excellent communication, stakeholder engagement, and relationship-building skills, with the ability to work effectively with participants, families, clinicians, staff, and external providers.
  • Strong computer literacy, documentation and record management skills, sound judgement and problem-solving ability, and a current National Police Check (or ability to obtain) and Driver Licence.

Desirable:

  • Previous experience in Support at Home, Home Care Packages, CHSP, or similar community care programs.
  • Experience using electronic clinical management and documentation systems.
  • Proven ability to work effectively within multidisciplinary teams across multiple locations or service streams.
