About the Role 

The Care Coordinator is responsible for coordinating residents’ person-centred clinical care in line with policies, procedures and professional standards, and in collaboration with the Care Manager.

The primary of purpose of the Care Coordinator is to focus on individual resident needs, ensuring that they have a current documented Care Plan, which meets their needs, and is reviewed and delivered by the multi-disciplinary care team.

 

No two days are the same! As a Care Coordinator, you will: 

  • Coordinate resident care with the Care Manager, including admissions and assessments.
  • Develop and review person-centred care plans with the care team and families.
  • Provide and oversee clinical care, including ADLs, medications, wound care, and observations.
  • Maintain accurate resident documentation, including care plans, charts, and incident records.
  • Monitor resident condition and escalate changes, including falls, wounds, and deterioration.
  • Support infection control and implement risk reduction strategies.
  • Supervise and support care staff, including allocation, guidance, and training.
  • Ensure duty of care by following policies, responding to incidents, and advocating for residents.

 

About You 

You’ll thrive with us if you have:

  • Bachelor of Nursing and Current Australian Health Practitioner Regulation Agency (AHPRA) registration
  • An understanding of the aged care accreditation process
  • Experience of working cooperatively with multi-disciplinary care teams
  • Strong clinical assessment, documentation and practice skills
  • Effective communication and interpersonal skills
  • Proficient computer and information management skills, including ability to use Microsoft and clinical software systems.
