We are seeking a compassionate and experienced Care Manager to join our dedicated team on a full-time basis at our Toogoolawah Aged Care Facility.

About the Role

As Care Manager, you will be responsible for overseeing and coordinating the delivery of exceptional, person-centred care to our residents. You will work closely with the General Manager and clinical team to ensure the highest standards of quality, safety, and wellbeing are maintained.

In this role, you will:

• Lead, mentor, and support a passionate team of nurses and care staff.

• Oversee resident assessments, care planning, and care delivery.

• Ensure compliance with Aged Care Quality Standards and regulatory requirements.

• Build strong relationships with residents, families, and healthcare partners.

• Contribute to quality improvement, infection control, and risk management processes.

• Champion a culture of respect, empathy, and excellence in all aspects of care.

About You

You are an experienced aged care professional with a strong clinical background and a genuine commitment to supporting older Australians to live with dignity and purpose.

You will bring:

• Bachelor of Nursing and current AHPRA registration.

• Proven experience in residential aged care and team leadership.

• Sound understanding of Aged Care Quality Standards and compliance requirements.

• Excellent communication, interpersonal, and problem-solving skills.

• Strong organisational and documentation skills.

• A collaborative, values-driven approach.
