About the Role

You’ll build meaningful, one-on-one relationships with participants and their families, tailoring support to individual needs while promoting independence, dignity, and wellbeing. From assisting with daily living and social engagement to supporting clinical and nursing care, every interaction matters. As a Care Partner – Regional Support (Home Care) based at our Launceston Office, Tasmania, you will deliver safe, person-centred, enablement-focused support that empowers participants to maintain independence, wellbeing, and strong connections to their community.

In this regional support role, you will build trusted and meaningful relationships with a dedicated cohort of participants, providing high-quality day-to-day assistance aligned with individual goals and Support at Home care plans. You will actively monitor participant wellbeing, respond to changes, and escalate concerns to the appropriate Clinical Team member or Manager when required.

Reporting to the Manager – Care Management Services – Home Care, this Monday to Friday position offers the opportunity to contribute to safe, respectful, and reliable service delivery within a supportive, purpose-driven team at Respect, while supporting participants across the region from our Launceston base.

No two days are the same! As Care Partner you will:

  • Deliver safe, person-centred and enablement-focused support that promotes participant independence, wellbeing and community connection.
  • Undertake comprehensive assessments and develop individualised care plans in partnership with participants and their representatives.
  • Conduct regular reviews, monitor changes in condition, and escalate risks or concerns in a timely manner.
  • Coordinate referrals and collaborate with clinical teams, allied health and external providers to achieve planned outcomes.
  • Support transitions to and from Restorative Care Pathways and contribute to End-of-Life and Palliative care planning in line with participant preferences.
  • Maintain oversight of allocated services and budgets to ensure alignment with care plans and funding requirements.
  • Foster professional, trust-based relationships with participants, families and key stakeholders, contributing to case conferences and risk management.
  • Ensure accurate documentation, compliance with legislative and quality standards, and active participation in continuous improvement and professional development.

About You

You’ll thrive with us if you have:

  • Minimum 3 years’ experience in aged care, home care, case management, or a related discipline, with solid knowledge of the Home Care and Aged Care sector, including the Aged Care Quality Code of Conduct and Standards.
  • Demonstrated ability to deliver participant-centred care, advocate effectively, and build strong relationships with participants, families, and stakeholders.
  • Proven capability to work independently, exercise sound judgement, prioritise tasks, and problem-solve in a fast-paced environment.
  • Experience managing budgets, monitoring funds, and ensuring services align with care plans and funding requirements.
  • Strong understanding of risk management, including hazard identification, incident reporting, and compliance with legislative and organisational requirements.
  • Proficient in computer and information management systems, with a current driver’s license and National Police Certificate (or ability to obtain).
