Home Care Case Manager – Launceston, Tasmania (Full-time)
We’re looking for a dedicated Home Care Case Manager to join our Launceston team. This is an exciting opportunity to make a real difference in the lives of older people in our community by supporting them to live well at home, with care that is tailored to their needs and goals.
As a Case Manager, you’ll be the key contact for a group of Home Care Package clients – guiding them through every stage of their journey with us, from intake to ongoing support. You’ll provide safe, person-centred, and enablement-focused care, helping clients to maximise their funding and achieve the best possible health and wellbeing outcomes.
Your role will include:
Meeting with new clients and supporting them through the onboarding process
Completing comprehensive assessments and developing individualised care plans
Working with families, health professionals and service providers to deliver quality care
Monitoring client needs, reviewing progress, and making timely adjustments when required
Managing budgets effectively to ensure funding is used in ways that enhance client wellbeing
Advocating for clients and ensuring care is always aligned with our values and the Aged Care Standards
This is a varied and rewarding position where you’ll balance professional expertise with compassion, advocacy, and problem-solving – all to ensure our clients feel respected, valued, and supported.
You’ll bring:
Experience in case management, aged care, home care, or a related field (typically 3+ years)
Knowledge of the aged care sector, including the Quality Standards and Code of Conduct
Strong communication and relationship-building skills
A client-focused approach, with confidence in advocacy and decision-making
Experience in budgeting and fund management
Solid IT and documentation skills
Current National Police Certificate (or ability to obtain) and driver’s licence
Desirable qualifications include: Certificate IV in Home & Community Care / Aged Care, or a relevant bachelor’s degree (e.g. Nursing).
