We are seeking a Clinical Care Coordinator to join our aged care home, Riverside Gardens in Nambucca Heads, NSW, where your experience and expertise will be highly valued.

Coordinate the admission of new residents, completing comprehensive assessments and developing person-centred care plans in collaboration with healthcare professionals.
Lead, mentor, and support the care team to deliver safe, high-quality clinical care and positive resident outcomes.
Provide hands-on nursing care, including medication administration, wound management, and ongoing clinical monitoring.
Maintain accurate clinical documentation, oversee infection control practices, and ensure compliance with regulatory standards and audits.
Communicate effectively with residents, families, and external healthcare providers while driving quality improvement initiatives.
You’ll thrive with us if you have:
Bachelor of Nursing with current AHPRA registration.
Minimum 2 years’ nursing experience in residential aged care, with experience in a similar clinical care role highly regarded.
Demonstrated ability to work collaboratively within multidisciplinary care teams.
Strong clinical assessment, documentation, and evidence-based practice skills.
Excellent communication and interpersonal skills to engage effectively with residents, families, and colleagues.
Proficient computer and information management skills, including Microsoft applications and clinical software systems.
Sound understanding of the aged care accreditation process and ability to meet Aged Care Worker Screening legislative requirements.
