About the Role

Make your clinical expertise count where it matters most — supporting people to live well, stay independent and achieve their goals at home. Based in Wangaratta, this full-time Clinical Care Partner role is an opportunity to use your clinical expertise in a meaningful, person-centred way. As a registered health professional, you’ll provide safe, clinically informed care to participants with identified clinical needs, completing comprehensive assessments, developing individualised treatment and reablement plans, and delivering targeted clinical nursing services that promote wellbeing, restorative care and functional independence. Working within your scope of practice, AHPRA registration and Support at Home requirements, you’ll build meaningful relationships with participants and their families while delivering care that reflects their individual needs, choices and goals.

This is more than a traditional clinical role — it’s an opportunity to provide clinical oversight, intervention, advice and leadership. You’ll work closely with Care Partners, the broader clinical team, GPs, allied health professionals and external health providers to deliver coordinated, evidence-based care. You’ll manage a portfolio of participants requiring clinical support and play a key role in Restorative Care Pathways, providing short-term clinical responsibility and supporting a smooth transition back to the Care Partner for ongoing case management. No two days are the same! As Clinical Care Partner – Home Care you will:

  • Deliver safe, person-centred clinical care that promotes wellbeing, reablement and independence.
  • Complete clinical assessments and develop individualised care and reablement plans.
  • Provide clinical nursing interventions, including wound care, chronic disease, medication, pain, skin and continence support.
  • Monitor clinical needs, risks and changes in condition, escalating and documenting as required.
  • Manage a clinical portfolio and coordinate referrals with GPs, allied health and other health providers.
  • Lead Restorative Care Pathways and support End-of-Life and palliative care.
  • Provide clinical guidance, coaching and delegation support to Care Partners.
  • Maintain clinical governance, documentation, compliance, participant advocacy and ongoing professional development.

About You

You’ll thrive with us if you have:

Essential

  • Current and unrestricted AHPRA registration (RN/EN).
  • Strong clinical judgement, communication and problem-solving skills, with the ability to work independently and manage competing priorities.
  • A person-centred approach, with a commitment to participant dignity, choice, advocacy and quality of life.
  • Understanding of clinical risk, escalation, incident reporting and safe practice.
  • Ability to build effective relationships with participants, families, colleagues and external health providers.
  • Sound digital and documentation skills, including client management systems and electronic records.
  • Current driver’s licence, National Police Certificate, First Aid and CPR, or ability to obtain where required.

Desirable

  • Experience in aged care, home care, community health or case management, ideally 3+ years.
  • Knowledge of the Aged Care Quality Standards, Code of Conduct and accreditation requirements.
  • Experience with care budgets, funds management or care-related financial information.
  • Experience identifying the need for clinical intervention, escalation and referral.
  • A genuine passion for aged care, reablement and improving participant outcomes.
  • Ability to embrace change, new models of care, systems and processes and contribute positively to a growing service.
