About the Role

Clinical management is central to support Home Care. It provides the structure to ensure care is accurately assessed, planned and continuously reviewed, bringing together clinical and allied health inputs into a cohesive, well-governed approach.

This ensures care delivery remains clinically sound while still aligning to the individual’s goals, preferences and level of independence — balancing risk, autonomy and safety in a way that is both considered and evidence-based.

We are seeking an experienced Clinical Manager to join our Home Care team in a pivotal leadership role supporting the delivery of high-quality, evidence-based clinical and allied health services.  This is a full-time maternity leave position (Monday to Friday) based in Tasmania, with flexibility to be located at our Lindisfarne or Launceston office.

Working closely with the Case Management team, you will help ensure services are responsive to client needs and aligned with organisational policies, supporting a consistent and client-centred approach to care. Through collaboration with clients, their representatives and key stakeholders, you will contribute to care that is coordinated, holistic and focused on wellbeing.

You will also play an important role in strengthening relationships with external providers, helping to establish integrated care pathways that enhance continuity of care for clients living at home.

With a strong focus on clinical governance, this position contributes to maintaining quality standards through audit oversight, compliance, and the ongoing development of policies, quality indicators and governance frameworks that support best practice across clinical care.

No two days are the same! As Clinical Manager – Home Care you will:

Clinical Leadership and Execution

  • Lead comprehensive client assessments, ensuring they are timely, best practice aligned and regularly reviewed, with oversight of ongoing monitoring, documentation, updates and communication with Case Managers and multidisciplinary teams
  • Support development and review of clinical care plans, ensuring they remain responsive to client needs, with timely referrals implemented while upholding client choice, dignity of risk and autonomy
  • Lead management of high-risk or complex clients, including case conferencing, participation in multidisciplinary meetings and high-risk forums, and oversight of clinical review processes
  • Own audit and governance activities, including the monthly audit schedule, reporting, compliance oversight, and contributing to continuous improvement, policies, procedures and best practice frameworks
  • Oversee referrals and build strong relationships with external providers, supporting integrated, coordinated care pathways aligned with HCP guidelines
  • Lead culturally appropriate care and inclusive practices, supporting diverse client groups and promoting diversity across service delivery
  • Lead and support clinical staff through mentoring, supervision, training and development, while contributing to professional development, research, data collection and service improvement initiatives

Duty of Care

  • Ensure clinical practice aligns with organisational policies, Aged Care Quality Standards, Code of Conduct, Home Care Packages Program and all regulatory and best practice requirements
  • Provide clinical leadership and oversight of client care, including timely responses to deterioration, guidance to Case Managers and teams, and delivery of client-centred, evidence-based care
  • Ensure effective incident, hazard and SIRS reporting in line with organisational and HCP requirements, while promoting a proactive safety and risk management culture
  • Advocate for client rights, wellbeing and autonomy, embedding person-centred care, dignity of risk and participation in decision-making alongside continuous improvement and quality outcomes
  • Oversee clinical resources and service delivery, including management of supplies and equipment, ensuring compliance, availability and safe use
  • Participate in on-call arrangements and undertake additional duties as required

Professional Development

  • Maintain current professional registration and advise of any changes to registration status
  • Keep practice current through evidence-based review and ongoing professional development in clinical care, governance, leadership and aged care
  • Maintain compliance with AHPRA requirements, mandatory training and Aged Care Quality and Safety Commission standards
  • Respond to feedback constructively and actively participate in performance reviews, meetings and required training
  • Maintain up-to-date knowledge of the home care sector, including industry developments, standards, issues and services

About You

You’ll thrive with us if you have:

Essential:

  • Current registration as a Registered Nurse with AHPRA with no restrictions.
  • Strong clinical assessment skills with a commitment to evidence-based, best practice care
  • Excellent communication skills with the ability to build strong relationships with internal and external stakeholders
  • Client-focused approach, with a genuine commitment to advocating for positive outcomes
  • Ability to work autonomously, make sound clinical judgments, and prioritise effectively in a fast-paced environment
  • Understanding of risk management protocols, including hazard identification and incident management
  • Proficient in computer systems and information management
  • Strong problem-solving and conflict resolution skills
  • Current National Police Certificate (or willingness to obtain)
  • Current driver’s licence

Desirable:

  • 5+ years’ experience as a Registered Nurse
  • Previous senior or leadership experience within home care, residential aged care, or health services
  • Knowledge of the aged care sector, including Aged Care Quality Standards and accreditation processes
  • Understanding of continuous improvement principles, or willingness to develop in this area
