About the Role

As an Occupational Therapist, you will play a key role in delivering high-quality, evidence-based care to older adults living in the community, with the opportunity to see the direct impact of your work every day.

Working within a supportive multidisciplinary team and reporting to the Clinical Manager, you will help clients maintain their independence and safety through tailored assessments, goal-focused interventions, and environmental modifications. Your work will enable older people to remain living confidently in their own homes and continue engaging in the activities that matter to them.

You will work closely with clients, families, Case Managers, and allied health professionals to deliver coordinated, person-centred care aligned with individual goals and preferences.

This is a rewarding opportunity to join a supportive and collaborative team as a full-time Occupational Therapist, based in either Devonport, Launceston, or Hobart. We are seeking a compassionate and motivated clinician with a strong client-centred approach, excellent communication skills, and a genuine passion for supporting older adults.

No two days are the same! As Occupational Therapist you will:

  • Conduct comprehensive assessments and maintain accurate, compliant documentation
  • Develop, implement, and review individualised therapy plans to support outcomes
  • Prescribe and monitor assistive technology, equipment, and home modifications
  • Monitor client progress and respond to changes, escalating concerns as needed
  • Coordinate referrals and collaborate with multidisciplinary and external providers
  • Provide education and practical support to clients, carers, and support staff
  • Contribute to care planning, case conferences, and client-centred service delivery
  • Support risk management, compliance, and continuous improvement activities

About You

You’ll thrive with us if you have:

Essential

  • Current AHPRA registration as an Occupational Therapist
  • Strong clinical reasoning and assessment skills, with a commitment to evidence-based, person-centred care
  • Experience in community, aged care, or rehabilitation settings, supporting independence and wellbeing
  • Excellent communication skills, with the ability to build rapport and collaborate across multidisciplinary teams
  • Demonstrated ability to manage a caseload, prioritise effectively, and make sound clinical decisions
  • Solid understanding of risk management, including safe work practices in home and community settings
  • Strong organisational, problem-solving, and time management skills in a dynamic environment
  • Current National Police Check and unrestricted driver’s licence

Desirable

  • 2–3 years’ experience as an Occupational Therapist in home care, community, or aged care
