Role Description
The Allied Health Assistant, Wellness & Reablement Services in partnership with Allied Health professionals, will support the delivery of community-based rehabilitation services. This role involves assessing, planning, implementing, and evaluating client programs. The program’s purpose is to address the effects of aging, and/or health conditions, focusing on enhancing the client’s overall well-being and independence.
Reporting Relationship
On a day-to-day basis, the Allied Health Assistant, Wellness & Reablement Services - Home Care shall report to the Team Leader, Wellness & Reablement Services - Home Care.
Qualifications & Experience
Essential:
- Certificate III in Allied Health Assistance or equivalent or demonstrated knowledge and experience in Community Services or Allied Health Assistance.
- Sound knowledge of Home Care and/or Aged Care industry including exposure to the Aged Care Quality Code of Conduct, Aged Care Standards & accreditation process.
- Strong communication skills, with a demonstrated ability to build rapport with internal and external stakeholders.
- Client-centric approach and willingness to advocate on behalf of clients.
- Proven ability to work independently, make sound judgements, and prioritise tasks in a fast-paced environment.
- Ability to work in a physical environment, including assisting with manual handling.
- Ability to follow instructions and work within a team under the supervision of qualified Allied Health professionals.
- Demonstrated ability to maintain confidentiality and adhere to privacy laws.
- Demonstrated understanding of risk management protocols, covering hazard identification and incident management.
- Demonstrated understanding and ability to act quickly and effectively in the event of an emergency, including administering first aid, providing CPR if necessary, and performing pool rescues (in a Hydrotherapy centre environment).
- Proficient swimming skills, and a willingness and ability, to undertake pool lifesaving training to a satisfactory standard (in a Hydrotherapy centre environment).
- Proficient computer and information management skills.
- Current National Police Certificate (or ability to acquire).
Desirable:
- Certificate IV in Allied Health Assistance.
Key Duties & Responsibilities
- Support the Wellness & Reablement team in their daily operations to ensure high quality service delivery is provided to our service users and delivered in accordance with our contract requirements.
- Deliver planned therapy sessions as directed by Allied Health professionals in a range of environments including within client homes, allied health centres, hydrotherapy centres or relevant communal locations, as required.
- Engage with clients on a holistic level about their performance and progression towards goals or concerns.
- Provide allied health therapy interventions as directed by relevant allied health professional.
- Monitor clients during therapy sessions ensuring exercises are performed safely and correctly, within scope of position, report any concerns or changes to the relevant Allied Health Professional and document.
- Modify a session or exercise within limits prescribed by treating Allied Health Professional.
- Respond to incidents, feedback, and complaints, in a timely manner and provide guidance and support to staff.
- Actively participate in client assessments and evaluations and collaborate with Allied Health professionals.
- Actively participate in ongoing quality improvement activities and ensure the team is supported to maintain the service line Continuous Improvement plans.
- Actively participate in working towards the Aged Care Quality and Safety Standards.
Key Duties & Responsibilities - Duty of Care
- Adhering to policies, procedures, and professional standards, including the Aged Care Quality Standards the Aged Care Quality Code of Conduct and Standards.
- Respond promptly and appropriately to client deterioration.
- Ensure environment and equipment is set up in safe, clean, hygienic, and functional way and is maintained, prior to, during and after client use.
- Act, report, and document hazards, incidents, or SIRS as per Respect Incident Management policies and procedures and Home Care Package Guidelines.
- Advocate for the rights and well-being of clients, ensuring that their dignity of risks is managed appropriately.
- Ensure client experience and satisfaction, focused on prompt complaint resolution, open disclosure.
- Contribute to continuous improvement initiatives and safety culture, facilitate, and participate in client conferences.
