From assisting with daily living and social engagement to supporting clinical and nursing care, every interaction matters. We’re seeking a compassionate and reliable Care Partner to support people to live independently, stay connected, and thrive in their own homes. Based in Wangaratta and across surrounding areas, delivering person-centred care that makes a genuine difference every day.

You’ll build meaningful relationships and become a trusted part of each client’s care journey, providing consistent, respectful support that focuses on individual choice and wellbeing. We’re looking for someone who is naturally empathetic, communicates with warmth and professionalism, and takes pride in being dependable and organised within structured care environments.

No two days are the same! As Care Partner you will:

  • Deliver safe, person-centred, enablement-focused case management that supports participant independence, wellbeing, and community connection.
  • Conduct comprehensive assessments and develop individualised care plans in partnership with participants, ensuring goals reflect strengths, preferences, and enablement outcomes.
  • Monitor participant wellbeing, identify and document changes or risks, and escalate appropriately to clinical teams or management.
  • Complete regular care plan reviews, reassessments, and updates in line with Support at Home requirements and changes in participant needs.
  • Coordinate referrals and collaborate with clinical staff, allied health, and community services to support holistic participant outcomes.
  • Manage care transitions, including restorative, palliative, and end-of-life pathways, ensuring clear communication and continuity of care across teams.
  • Oversee service coordination, participant budgets, and workforce scheduling alignment to ensure safe, consistent, and compliant service delivery.
  • Maintain accurate documentation, uphold legislative and organisational requirements, and contribute to risk management, complaints handling, and continuous improvement.

About You

You’ll thrive with us if you have:

  • Minimum 3+ years’ experience in case management and/or aged care, home care, or a related field.
  • Strong understanding of the aged care/home care sector, including Aged Care Quality Standards, Code of Conduct, and accreditation processes.
  • Excellent communication and interpersonal skills with proven ability to build effective relationships with stakeholders.
  • Participant-focused mindset with a strong commitment to advocacy, dignity, and person-centred care.
  • Ability to work independently, exercise sound judgement, and prioritise effectively in a fast-paced environment.
  • Experience in budgeting, funds management, and service coordination.
  • Demonstrated knowledge of risk management, including hazard identification and incident reporting.
  • Strong digital literacy, problem-solving ability, and conflict resolution skills.
  • Registered Nurse with AHPRA registration is desirable but not essential
