At Respect, our Independence at Home services help older Australians remain safe, comfortable, and connected in the place they value most — their own home. You’ll build meaningful, one-on-one relationships with participants and their families, tailoring support to individual needs while promoting independence, dignity, and wellbeing. From assisting with daily living and social engagement to supporting clinical and nursing care, every interaction matters. Join Respect as a Care Partner – Regional Support and be part of a team delivering meaningful, person-centred care across regional communities.

Based out of our Launceston office, this additional role has been created to support ongoing organisational growth. Working Monday to Friday, 8:30am to 4:30pm, you’ll operate in a multi-region environment with the flexibility to work remotely while staying connected to the people you support.

In this role, you will build trusted relationships and make a genuine difference in people’s lives by supporting care that promotes independence, dignity, and choice.

No two days are the same! As Care Partner – Regional Support you will:

  • Deliver person-centred, enablement-focused case management to clients across multiple regions using remote and virtual engagement.
  • Complete assessments and develop individualised goals and care plans aligned with client needs, preferences, and Support at Home requirements.
  • Monitor client wellbeing through regular contact, identifying changes, risks, or emerging needs and escalating as required.
  • Coordinate services, referrals, and multidisciplinary support to promote independence, wellbeing, and continuity of care.
  • Manage care transitions, including handovers to Clinical Care Partners or local teams and integration of clinical recommendations into care plans.
  • Collaborate with internal teams and external providers to ensure consistent, reliable, and coordinated service delivery across regions.
  • Maintain accurate documentation, complete care reviews and reassessments, and ensure compliance with organisational and legislative requirements.
  • Build strong relationships with clients and stakeholders while advocating for dignity, cultural safety, inclusion, and positive client outcomes.

About You

You’ll thrive with us if you have:

  • Minimum 3 years’ experience in case management and/or aged care, home care, or a related field.
  • Sound knowledge of the aged care sector, including Aged Care Quality Standards, Code of Conduct, and accreditation processes.
  • Strong communication and interpersonal skills, with the ability to build rapport with a range of stakeholders.
  • Participant-focused approach, with a commitment to advocacy, dignity, and person-centred care.
  • Ability to work independently, exercise sound judgement, and manage competing priorities in a fast-paced environment.
  • Experience in budgeting, funds management, and service coordination.
  • Demonstrated understanding of risk management, including hazard identification and incident response.
  • Proficient computer skills, with the ability to manage documentation, problem-solve, and navigate systems effectively.
