We are seeking a Clinical Care Coordinator to join our aged care home, Bayview Haven in Portland, Victoria, where your experience and expertise will be highly valued.

This full-time role offers the chance to bring your clinical knowledge to a professional, supportive team and play a meaningful role in delivering safe, effective, and person-centred care. You will have the opportunity to mentor and guide nursing staff, contribute to care planning, and collaborate closely with residents, families, and management, making a real difference in the quality of care provided. No two days are the same! As Clinical Care Coordinator you will:

  • Coordinate the admission of new residents, completing comprehensive assessments and developing person-centred care plans in collaboration with healthcare professionals.
  • Lead, mentor, and support the care team to deliver safe, high-quality clinical care and positive resident outcomes.
  • Provide hands-on nursing care, including medication administration, wound management, and ongoing clinical monitoring.
  • Maintain accurate clinical documentation, oversee infection control practices, and ensure compliance with regulatory standards and audits.
  • Communicate effectively with residents, families, and external healthcare providers while driving quality improvement initiatives.

About You

You’ll thrive with us if you have:

  • Bachelor of Nursing with current AHPRA registration.
  • Minimum 2 years’ nursing experience in residential aged care, with experience in a similar clinical care role highly regarded.
  • Demonstrated ability to work collaboratively within multidisciplinary care teams.
  • Strong clinical assessment, documentation, and evidence-based practice skills.
  • Excellent communication and interpersonal skills to engage effectively with residents, families, and colleagues.
  • Proficient computer and information management skills, including Microsoft applications and clinical software systems.
  • Sound understanding of the aged care accreditation process and ability to meet Aged Care Worker Screening legislative requirements.
