About the role
The Clinical Care Coordinator is responsible for leading and coordinating high-quality, person-centred care in line with best practice and organisational standards. Working closely with the Care Manager, this role ensures residents have up-to-date Care Plans tailored to their individual needs and oversees their implementation with the support of a multidisciplinary team.
