Join a purpose-driven organisation delivering high-quality home care and community services. We’re seeking a compassionate Care Partner – Regional Support to provide person-centred case management to clients across multiple regions through a remote service delivery model.
In this role, you’ll support clients with low to moderate needs to live safely, confidently, and independently at home. You’ll build strong relationships through regular virtual contact, coordinate services, monitor wellbeing, and ensure seamless transitions when face-to-face support is required.

Deliver safe, person-centred, wellness-focused case management via virtual communication.
Assess clients’ functional, social, environmental, and wellbeing needs.
Develop individualised care plans and goals that promote independence and choice.
Provide proactive monitoring through regular check-ins, identifying early changes in client condition.
Escalate risks or emerging needs to Clinical Care Partners or Managers.
Coordinate referrals to clinical staff, allied health, community services, and external providers.
Support seamless participation in Restorative Care Pathways, including handover and follow-up.
Collaborate with scheduling and workforce teams to ensure reliable service delivery.
Maintain accurate and compliant documentation and care plan updates.
Participate in multidisciplinary discussions, case conferences, and high-risk reviews.
Provide culturally safe, inclusive support to CALD, ATSI, LGBTQIA+, and other diverse communities.
Essential:
3+ years experience in case management or aged/home care support.
Strong knowledge of Aged Care Standards, the Aged Care Quality Code of Conduct, and the home care sector.
Excellent communication and rapport-building skills in a virtual environment.
Participant-centred approach with advocacy skills.
Ability to manage a high-volume caseload independently and prioritise effectively.
Experience with budgeting and home care funds management.
Understanding of hazard identification, incident reporting, and risk management.
Competency with client management systems and confident computer skills.
Sound judgement, problem-solving, and conflict resolution skills.
Current National Police Certificate (or ability to obtain).
Valid Driver Licence.
Desirable:
Certificate IV in Home & Community Care, Aged Care, or similar.
Bachelor’s degree (RN desirable but not required).
Exposure to workforce planning or scheduling systems.
