About the Role:  Join our dedicated team at Respect Chaffey House (Merbein VIC) as a Clinical Care Coordinator, where you will play a key leadership role in delivering exceptional, person-centered care to our residents.

In this full-time position, you will lead clinical governance across the home, ensuring care is safe, effective, and aligned with best practice standards. You will coordinate and oversee comprehensive care planning, supporting residents with complex clinical needs while promoting dignity, independence, and wellbeing.

Working closely with a multidisciplinary team, you will provide clinical leadership, guidance, and mentorship to nursing staff, fostering a culture of continuous improvement and professional development. You will also ensure compliance with aged care standards and legislation, while building strong relationships with residents, families, and the broader care team. No two days are the same! As Clinical Care Coordinator you will:

  • Coordinate the admission of new residents, completing comprehensive assessments and developing person-centered care plans in collaboration with healthcare professionals.
  • Lead, mentor, and support the care team to deliver safe, high-quality clinical care and positive resident outcomes.
  • Provide hands-on nursing care, including medication administration, wound management, and ongoing clinical monitoring.
  • Maintain accurate clinical documentation, oversee infection control practices, and ensure compliance with regulatory standards and audits.
  • Communicate effectively with residents, families, and external healthcare providers while driving quality improvement initiatives.

To Be successful you will require:

  • Bachelor of Nursing with current AHPRA registration.
  • Minimum 2 years’ nursing experience in residential aged care, with experience in a similar clinical care role highly regarded.
  • Demonstrated ability to work collaboratively within multidisciplinary care teams.
  • Strong clinical assessment, documentation, and evidence-based practice skills.
  • Excellent communication and interpersonal skills to engage effectively with residents, families, and colleagues.
  • Proficient computer and information management skills, including Microsoft applications and clinical software systems.
  • Sound understanding of the aged care accreditation process and ability to meet Aged Care Worker Screening legislative requirements.
